Teriparatide
Also known as PTH(1-34) · Forteo · Forsteo
The 1–34 fragment of human parathyroid hormone — the first bone-building (anabolic) osteoporosis medicine.
In 60 seconds
Approved for severe osteoporosis in many countries (prescription-only). Reviews document hypercalcaemia and dizziness; an osteosarcoma signal in rats led to historical duration limits in labelling.
- VERO compared new fractures with teriparatide versus risedronate in severe postmenopausal osteoporosis.
- DATA compared bone-mineral-density gains with teriparatide, denosumab or both combined.
- Reviews teriparatide’s unique bone-forming mechanism compared with antiresorptive agents.
Mechanism
Intermittent, once-daily activation of the PTH1 receptor stimulates osteoblasts more than osteoclasts, building new bone; continuous PTH exposure does the opposite. This timing-dependent effect is why teriparatide is anabolic rather than resorptive.
Research status
Approved for severe osteoporosis in many countries (prescription-only). Head-to-head fracture trials (VERO) and combination studies with denosumab (DATA) are its landmark data.
