SS-31 (Elamipretide)
Also known as elamipretide · Bendavia · MTP-131 · Forzinity
A mitochondria-targeted tetrapeptide that binds cardiolipin in the inner mitochondrial membrane — approved in the US in 2025 for Barth syndrome.
In 60 seconds
Accelerated US approval (September 2025) to improve muscle strength in Barth syndrome; further trials in other mitochondrial disorders. Clinical programmes report injection-site reactions as the most common effect.
- Summarises the September 2025 US accelerated approval of elamipretide for muscle strength in Barth syndrome.
- Reviews elamipretide’s structure, cardiolipin-binding mechanism and research applications.
- In an ischaemic kidney model, SS-31 preserved cristae and speeded ATP recovery by interacting with cardiolipin.
Mechanism
SS-31 concentrates in the inner mitochondrial membrane and binds cardiolipin, the lipid that holds the electron-transport chain together. This is reported to stabilise cristae structure and restore ATP production in injured mitochondria.
